About the Author
Trevor Kletz, OBE, D.Sc., F.Eng., a process safety consultant, has published more than a hundred papers and nine books on loss prevention and process safety, including most recently Lessons From Disaster: How Organizations Have No Memory and Accidents Recur and Computer Control and Human Error. His experience includes thirty-eight years with Imperial Chemical Industries Ltd., where he served as a production manager and safety adviser in the petrochemical division, and membership in the department of chemical engineering at Loughborough University, Leicestershire, England. He is currently senior visiting research fellow at Loughborough University and an officer of the Order of the British Empire.

Great book for both new and old process industry folks alike. I bought this book the week I got my first job offer from an oil company, and it's been on my shelf (in various editions) ever since. Great summary of things to watch out for, common mistakes that have nailed us in the past.
This is the book you buy & give to the new hires as Required Reading before you let them tough anything.

Examining 'What went wrong' 5th Ed

I had read the other two recent books by Kletz, 'Still going wrong' and 'Learning from accidents'. The style is easy to read and the stories are really fascinating if you are into safety and industrial chem. The only negative really is that the three books contain similar content to each other in that they overlap with the same stories for say 50% of the items. Regardless, this particular volume WWW 5th Ed is an extremely good reference tool for safety and process professionals.

Serves its purpose beautifully

I have loved all their atlas's. The book had all the basic info you look up all the time and its in one spot without searching the web and I love the detailed city maps. People seem to complain that its not something it was never intended to be. Sure you can get a lot of the into all over the web. I prefer to pull out my atlas's to quickly and concisely find what I need. The travel routes alone were worth it to me! Large hardbound color book fro $13? Its awesome!

Worth every penny

I love the atlas, its a great tool. When it came it was very well made. If you are thinking about purchasing this do it, its a great product. Everyone else is just upset it took so long to ship.

With digging you would be able to find the information on the internet but this is made for you, and its on nice paper, and nicely bound. The book even smelled fresh when it came. I mean I opened it and it woof.. freshly processed paper. My teenage brother loves it too, maybe I'll get him one.

With that said, it is a good introduction into the area
of EDP Auditing-particularly for college students. It covers
the management framework in the data center, systems development,
the program life cycle, database administration, operations
management, data capture, audit trail controls, evidence
collection and evaluation. The work is written from the EDP
professional's point of view. This text would complement a traditional auditing course by covering the EDP Audit component. It is a bit weak in the telecommunications area; however, the thrust of the work provides a solid foundation in the theory and practice of EDP Auditing for students studying the area for the first time. This book should be taught with a supplemental class project to help students understand how this theory is applied. I would suggest that the author consider an update of the current version to cover more theory of network administration.

A good primer

This book may be outdated, but gives a very good overview of the edp audit discipline. I notice a very critical review of this book but I can submit as a IT Risk Management professional that audit is all about controls and hence there would be mechanisms for checks on data entry or any other IT personnel. Traditional auditors look for controls on business processes and EDP auditors look for controls in IT environment. So, I hope the reader looks at it from that perspective and there is no insult in this case. Its like saying that Police should trust every body and feel that there are no criminals at all. Then why police in the first place?
In conclusion, this is a good book for those who are not very much into IT but are planning to move into this new profession. However for advanced IT professionals, there are other books on IT Audit that they can refer.

This book is so out of date, it commonly refers to the advent of the minicomputer and its potential effects on the business world. Its filled with ot of date theories, and it takes a down right insulting view of computer professionals. It goes as far as saying that data entry personel are unreliable and should not be trusted!

Morihei Ueshiba is rightly known as one of the world's greatest martial artists for his creation of aikido, the synthesis of his superb technique and his profound spiritual insight into the nature of the universe. In 1938 Morihei wrote Budo (Martial Way) as an instruction manual and a statement of the aims and spirit of his martial arts philosophy. Budo, however, was never made available to the general public, and it even contains the injunction that "these techniques ... should not be disclosed indiscriminately to the public."


Now with the worldwide popularity of aikido rivaling that of judo and karate-do, the Aikikai Foundation and the Ueshiba family have decided that it is time for this historic treatise of the martial arts to be translated into English, and made available to general readers and students of aikido all over the world. Reproduced faithfully from the original, this seminal work contains one of the founder's few personal statements of his basic principles, expressed both in prose and mystical poetry. The text is accompanied by photographs of Morihei illustrating the aikido techniques he describes.


This martial arts classic is preceded by an introductory biographical essay on Morihei by his son Kisshomaru Ueshiba, the present chairman of the Aikikai Foundation. This enlightening work recounts the founder's early life, his inspirational 'encounter with the religious mystic Onisaburo Deguchi, and the spiritual revelations that led him to develop the art of aikido.


The volume concludes with a series of action photographs of the founder demonstrating aikido techniques, taken at a private dojo in Tokyo in 1936. This section also includes selected photographs of Morihei in action in 1951. The more than 400 historic photographs shown here provide an unequalled opportunity to observe the lifelong devotion to aikido and the continuity of performance that characterized Morihei's approach to his art.

About the Author
MORIHEI UESHIBA was born in Western Japan in 1883. After mastering classical styles of judo, kendo, and jujutsu, he created aikido, an entirely original martial art, based on the spiritual teachings of the Omoto-kyo religion. After the Second World War, Morihei established the Aikikai Foundation in Tokyo to promote the dissemination of aikido throughout the world. Morihei died in 1969.


KISSHOMARU UESHIBA is chairman of the Aikikai Foundation, the largest aikido organization in the world. He has written several books on aikido in Japanese and English, including the classic Spirit of Aikido.


The translator, JOHN STEVENS, is a professor of Buddhist studies and aikido instructor at the Tohoku Fukushi University in Sendai, Japan. He is a prolific writer on aikido and Buddhism, and has written a biography of Morihei Ueshiba entitled Abundant Peace.

The founder's foundation

The more you know about Aikido and what it really is, the better you will be at it. This book provides some of the most fundamental teachings of O'Sensei in words and pictures. Albeit, it is probably not ideal for the novice practitioner. Most of the sequences consist of 3 pictures. And, while educational, we have had a good laugh about a few of them: Frame 1- uke attacks. Frame 2- tori blocks, Frame 3- uke is on the ground across the dojo :). We are not quite sure how he got there.
But, it has provided hours of exploration and experimentation. If you have some Aikido experience, you can generally work out the technique which is always very subtle and very elegant. I believe even the most practiced aikidoka (especially the most experienced aikidoka?) could learn from this text.

Informational teachings and history by the master.

The history and teachings are very informational if your interested in how master O'sensie started budo, bujutsu, aikido. His teachings of the warrior path and spirit and the arts is very enlightening. Love the book i find the teaching and spirituality more then what was expected, if your a martial artist interested in the history of budo/aikido i have found so far no other book as informational.

I really liked the book as someone who tried Aikido and quit ...

I really liked the book as someone who tried Aikido and quit because of other commitments I had, it shows Ueshiba having abilities from other maritial arts combining them together. Pre WW2 Aikido was more aggressive with use of Atemi waza ( my opinion) today's Aikido in some circles do not use atemi. He even says in a real fight Aikido is 70% atemi, even still, it was a nice read.

About the Author

Hongji Yang is Leader of SERG at the Software Technology Research Laboratory at De Montfort University, UK. He serves in the Department of Computer Technology. His research interests include software engineering and distributed computing. He teaches Software Architecture and Software Evolution. He served as a programme co-chair at IEEE International Conference on Software Maintenance 1999 and as programme chair at IEEE Computer Software and Application Conference 2002.

Xiaodong Liu received his PhD in Computer Science from De Montfort University, UK. He is a reader and the director of Centre for Information & Software Systems, in the School of Computing, Edinburgh Napier University, UK. As an active researcher, his current research focuses on Context-aware adaptive services, service evolution, mobile clouds, pervasive computing, software reuse, and component-based software engineering. Dr. Liu has led 6 externally funded projects, and published over 50 papers in established international journals and conferences and 2 book chapters. He is the inventor of 1 patent registered in UK, USA and at International Level. He has been the chair, co-chair or PC member of a number of IEEE and IASTED international conferences. He is the editorial board member of 3 international journals and editor of 2 research books. He is a member of IEEE Computer Society and British Computer Society.
