About the Author
Trevor Kletz, OBE, D.Sc., F.Eng., a process safety consultant, has published more than a hundred papers and nine books on loss prevention and process safety, including most recently Lessons From Disaster: How Organizations Have No Memory and Accidents Recur and Computer Control and Human Error. His experience includes thirty-eight years with Imperial Chemical Industries Ltd., where he served as a production manager and safety adviser in the petrochemical division, and membership in the department of chemical engineering at Loughborough University, Leicestershire, England. He is currently senior visiting research fellow at Loughborough University and an officer of the Order of the British Empire.

Eye Opener

Great book for both new and old process industry folks alike. I bought this book the week I got my first job offer from an oil company, and it's been on my shelf (in various editions) ever since. Great summary of things to watch out for, common mistakes that have nailed us in the past.
This is the book you buy & give to the new hires as Required Reading before you let them tough anything.

A must read for everyone who is involved in or has any relation to process plant design, operations, maintenance or management. Provides a deep understanding of the industry and its risks. The fifth edition has the second book Still Going Wrong from 2003 as part B. Simply great.

Examining 'What went wrong' 5th Ed

I had read the other two recent books by Kletz, 'Still going wrong' and 'Learning from accidents'. The style is easy to read and the stories are really fascinating if you are into safety and industrial chem. The only negative really is that the three books contain similar content to each other in that they overlap with the same stories for say 50% of the items. Regardless, this particular volume WWW 5th Ed is an extremely good reference tool for safety and process professionals.
